Acceptance

By using the services/products provided on the Gastropak Web site (this site), you agree that you have read and understand the term and conditions by which Gastropak operates and the responsibilities of both Gastropak and yourself and you agree to be legally bound by these terms and conditions. If you do not understand these terms and conditions then you should not use Gastropak services and you should not buy any products from this site.

Accounts

To be able to buy products from this site, you need to create an account with some personal details. You agree that you will not create more than one account and that you will not attempt to order products via more than one account. You also agree that you will not create any accounts for persons other than yourself to facilitate the order of products for a third party.

The Law

This site operates within and is governed by English Law and you agree to be bound by and submit to the jurisdiction of the English Courts.

Your Information

You agree that all information that you provide to Gastropak is current, true and is accurate. You agree that you are responsible for the provision of this information and that you will not omit information that could be relevant to the decision process for the prescription of medication or advice that is given by Gastropak. You agree that Gastropak will not be in any way liable for mis-prescription of medication as a result of you providing incorrect or omitting information. You agree that if you are in any way in doubt about what information you need to give, you will contact Gastropak to seek further advice and explanation before placing any order for medication. By providing this information you agree that you are giving your informed consent to Gastropak to prescribe medication contained in the Gastropak products.

Advice Given

Advice given to you by Gastropak (their medical experts) is provided on the basis of the information that you have provided. How you use this advice is your responsibility. You agree that Gastropak has provided enough advice for you to make an informed decision about the use of Gastropak products. You agree that if this is not the case you will seek further advice from Gastropak before commencing the treatments provided.

Prescribed Medicine

You agree that any medication that is prescribed to you by Gastropak is for your own personal use and that you will not pass it on to a third party. You also agree that you will ensure that you are fully aware of the method of taking the treatment and that if you are unsure in any way as to how or when you should use the treatment that you will not take the treatment until such time that you have contacted Gastropak for clear instructions. In addition to this, you agree that if you suffer any side effects from the treatment, that your primary contact will be with Gastropak to allow Gastropak to provide timely after service care.

After Care

You agree that you have been advised to let your own GP know of these prescribed medicines. You also agree to allow Gastropak the opportunity to provide primary aftercare in the event that such aftercare is needed. Failure to allow this opportunity will form an acceptance on your part that Gastropak is no longer the primary aftercare service and that Gastropak is no longer liable to or responsible for your health / well being and that you willingly have chosen an alternative aftercare provider.

Delivery

Gastropak will not be responsible for the loss of shipped goods that are signed for, but do not reach the person that placed the order. It is your responsibility to ensure that on signing for the delivery, the goods will be received by yourself of someone you have nominated. Gastropak will not be responsible for any costs if goods are not collected from or are returned to a post office due to there being no one at the delivery address to collect the delivery. Further to this, as this product cannot be re-used, you do not have the right to return this product. Therefore you must ensure that you are fully aware of and willing to be responsible for the placing of the order in the knowledge that once placed, there is no possibility of a return or a refund.

Privacy Policy

We keep all your personal information secret. We do not pass your data on to any third party, other than the Care Quality Commission on their request) and you will never receive spam mail as a result of Gastropak passing on your details. We do not sell, rent nor spam any email addresses processed by our service. Gastropak also agrees that all personal information will only be viewed by the GP and that this information will only be used for the decision to prescribe the medication in relation to the Gastropak products. Further to this, you are within your rights to ask for your information to be removed from our database at any time. You can use the contact form and simply request that your prescription form data be removed from the system. You can also make a similar request if you would like the whole of your account removing from the Gastropak database,

Changes to Our Terms

Our terms may, from time to time, be subject to change. You agree that on using our services you have checked for changes to the terms and conditions and that you agree with the terms and conditions as they are presented at the time you make your purchase or seek advice from Gastropak. You agree that you are solely responsible for ensuring that you have read and understood the terms and conditions at all given times.
